Please note that the benefits included in your membership, such as access to labs and discounts on products, are applicable for the duration of your one-year membership. Should you choose to cancel and have utilized any of these services, there may be a balance on your account. Our dedicated accounting team will promptly follow up to process any outstanding payments required to make the membership cancellation whole.

Once we receive back the form below, we will begin to process your cancellation. Please note that if you are canceling your membership between monthly billing cycles, the date of cancellation may not be 30 days from today.
