Follow these easy steps to start your consultation process!

Schedule a Consultation: To begin the enrollment process, schedule a consultation with one of our membership consultants. During the consultation, we will get to know you and your healthcare needs, and answer any questions you may have about our services.

Choose a Membership Plan: After or during the consultation, choose a membership plan that best fits your healthcare needs and budget. Our membership plans start at a low fee and include unlimited office visits, virtual consultations, and access to on-site diagnostic services and IV infusions.

Sign Up: Once you have chosen your membership plan, sign up with our office staff and meet with your physician for a consultation. You will be asked to provide some basic information, such as your name, address, and contact information.

Payment: After signing up, you will need to make your enrollment payment and your membership fee. You can do this online, or in person. We accept cash, checks, and credit cards.

Schedule Your First Appointment: Once you have completed the enrollment process, schedule your first appointment with your healthcare provider. During the appointment, the provider will conduct a comprehensive health assessment and develop a personalized care plan based on your unique healthcare needs.
